Qualifying purchase must be made between November 25, 2015 and December 31, 2015. Submissions must be post marked no later than

January 15

th

2016. A valid signature, a copy of the original receipt or invoice, the original UPC code(s) and Serial Number(s) cut from the product box along with this

form are required to process your rebate request. The rebate will only be mailed to the name and the address on the receipt or invoice you submit. Purchase must be

for consumer use only and not for resale or commercial purposes. Limit one (1) Rebate per category and one (1) rebate submission per household. Not available to

employees, dealers and distributors of Boss International Group, aka BIG. Packaging slips, credit card authorization slips, credit card statement or shipping documents

will not be accepted as proof of purchase in lieu of a copy of the original, dated sales receipt. No responsibility is assumed by BIG, its affiliates, agents, promotion

agencies, distributors, and/or dealers for Rebate documents not received by the specified time and at the specified address, regardless of reason, or for lost, stolen,
