SIX-MONTH PREVENTIVE MAINTENANCE RECORD Photocopy this form and use it to record the preventive maintenance performed on the treadmill. Each copy of the form can be used for six months (26 weeks). When maintenance is performed, write the date in the appropriate spaces. Make sure to perform each maintenance procedure as described on pages 22 to 23. If the procedures are not performed as described, components may wear excessively, the treadmill may be damaged, and the warranty will be voided. Weekly Maintenance Inspect and tighten all external parts of the treadmill. Clean the treadmill. Check the walking belt for proper tension and alignment.
